Purpose: This questionnaire aims to examine the servant leadership behaviors you exhibit and compare how you think of your leadership to how two of your colleagues or classmates think of it, as it relates to servant leadership.
Instructions: Read each item carefully and complete the questionnaire by applying the instructions below. Also, ask two colleagues or classmates to read each item carefully and use the following 7-point scale to indicate the extent to which they agree or disagree with the following statements as they pertain to your leadership. In these statements, “the leader” is referring to you in a leadership capacity.
